PA FBI Criminal History Record (Act 114 of the 2006 PA School Code)

To ensure that the documents attached to your application are the best quality possible, we advise all applicants to print and read the guidelines below.

Pennsylvania Residents ONLY:

1. Do not send your original clearance form.
2. Clearance results are valid for one year. You must renew your clearances annually and send PA-Educator.net a copy of the results.
3. PA FBI Criminal History Records older than one year have expired and will not be added to an applicant's credentials file.
4. Instructions and the clearance application form can be found at the PA Department of Education website
5. PA-Educator.net will accept a placeholder for the PA-FBI Criminal History Record. However, there are only TWO items that will be considered a placeholder:
1. A copy of your receipt from Cogent Systems indicating that you have paid the processing fee for your PA-FBI Criminal History Record, or
2. A Notarized Affidavit. To use the affidavit as a placeholder for your PA FBI Criminal History Record, print the form (below), complete it, have it notarized, and send the original notarized affidavit to PA-Educator.net.

Out-of-State Residents ONLY:
